 1. The card holder can avail upto 20% discount on tests. 

 2. Discounts are available for tests done within the centre and are not applicable for outsourced tests 

 3. Additional Charges for home sample collection. 

 4. The card is valid for the card holder and his family members. 

 5. Issue of duplicate card @ Rs.100/- 

 6. Misuse or fraudulent use of the card by any person including card holder may lead to cancellation of health card 

 7. In the event of any dispute, the decision of Samiksha Diagnostic is final. 

 8. Samiksha reserves the right to amend these terms and conditions without prior notice. 

 9. Please produce the card at the time of testing.
